those forms along with payment to the address below, or print

Standard
4 to 6 week delivery time. Orders over $100.00 may take

All checks and Money orders must be drawn on a United States bank. IE... Citibank, Bank of America, Chase ....

Shipments will be sent First Class Mail.
Overseas airmail requires an additional 20% surcharge.
Include $1.50 shipping and handling charge for all orders
$15.00 and under.
Include a 15% shipping and handling charge on all orders between
$15.00 and $50.00
Include a 10% shipping and handling charge on all orders over
$50.00
Packing
Small parts are not identified in standard packaging. If identification
is required, please add 25 cents per type of item.
